Need immediate police, fire or medical help in the United States? Call 911 first. Follow the dispatcher's instructions. Call if you can; text 911 if you cannot call, where available. Text-to-911 is not available everywhere. This worksheet does not provide emergency assistance.

Use these contacts for routine help and family updates. Complete your copy privately. Keep passwords, door codes, account numbers and medical details off this sheet.

Before you share and when details change

  • Ask each person first. Agree on the help they can offer and who may receive their details.
  • Check the numbers. Save them in your phone. Try an ordinary call or text together; do not call 911 simply to test it.
  • Keep a paper copy with you and one in an agreed place at home.
  • Share only with people who need it. A contact sheet contains personal information.
  • Review at least yearly and after a change in details or willingness to help. Update the review date and replace outdated copies.
